Examples of Program Changes that Should be Included in an Annual Report

 (PHS Policy IV.A.1.a.-i.)

  • the designation of a different individual as the Institutional Official;
  • administrative modifications that affect the reporting channels of the IACUC, the institutional official, or the veterinarian;
  • modifications to IACUC procedures;
  • change in the veterinarian designated as having program authority;
  • change in the veterinarian's authority or responsibility;
  • modifications to the occupational health program or the training program; and
  • changes in the species or approximate numbers of animals housed at the institution.
